because most of them have NATIONAL HEALTH programs that allow them HUGE consumer buying power in bulk purchasing.
whereas privatised insurance-based sets their premiums at highest billable uninsured retail, then contracts with providers for wayyyyyy less than billable prices. it's a wicked profit margin in between & they won't allow us to change/abolish that in the US.

absolutely. Terrible tragedy.

Got to see him do this tune, and most of the album, at about the best outdoor show I've ever seen--and I was like 20 ft away from him. I have never seen so much slack-jawed amazement on so many faces as when he started whacking that beatup old D-18 and making magic come out.
