They are always like that. We have one which is usually neat and clean, but most are just as you say.
And yet, America still shops there and makes light of the fact that this is another instance of a business model where one or two make it and the rest are fodder for the stock owners.

They apparently settled.
I am not surprised. By citing "health reasons" for her firing, they pretty much sealed their culpability.
Especially if it's an "employment at will" state, where you can be fired just because, so it's usually much harder to prove discriminatory intent because they don't have to give a reason.

I live in an "at will" state. This incident obviously was at fault.
Given that pregnancy is definitely a gender issue, one can hope that there's grounds for a discrimination suit.
It absolutely is grounds for a suit. It's even illegal to ask if someone is pregnant or planning to have children in a job interview. That's why I'm surprised they put down "for medical reasons."
