Had a routine medical appt this morning. Had to fill out forms which ask about your mental health, if you have money for rent/food, about possible spousal abuse etc. I don't have a problem with this. What I DID have a problem with was the young lady noticed I checked off that I didn't attend church. She then proceeded to invite me to her church. I sweetly thanked her and told her that that would never happen. Should this be something that is discussed in a medical setting?

Perhaps not, but on the other hand some people are at odd ends and may benefit from the fellowship and sense of belonging.

Not all churches are mired in hateful and divisive mythology. The Unitarian Univeralist church is a good example, so is the Satanic Temple.
