Provider (hospital, surgeon, etc. combined) bills Medicaid $15k, gets paid $9-11k.

Same exact surgery, level of difficulty, surgeon, hospital, etc., but billed to private insurance (Blue Shield, Aetna, etc.) for $135k, and ins co pays $99k.

That's what drives up the cost of insurance. The premium pool is based on those charges, not Medicaid charges.
