-Andrew Robinson refused to reprise his role as Larry Cotton, forcing hasty script rewrites. This partially accounts for the muddled story structure of the final film.

-The horn sound that is continually made by Leviathan is Morse code for "God".

-The film is included on the film critic Roger Ebert's "Most Hated" list.

Michael McDowell was originally slated to write and direct the movie but dropped out do to personal reasons. Instead it was directed by Tony Randel in his first cred as a director. Randel claims the change from a horror film (as the first movie was) to an dark cosmic horror fantasy was meant to differentiate the film from its predecessor. The dark tone of the movie, he says, reflected his own mindset on the world at the time.

Kenneth Cranham (Channard) claimed his involvement was due to his grandson pestering him to take up the offer being a fan of the original. At the time filming took place, he was doing a play in London with Gary Oldman. When Oldman heard Cranham was doing Hellbound: Hellraiser II (1988), he told him he would love to play a part in a horror movie. Years later, he starred in Bram Stoker's Dracula (1992). - IMDb

William Hope (Kyle) said that when his mother-in-law and nephew were in London, they asked if they could join him in one day of filming. Hope said yes, much for his surprise, because that particular day, they were filming the gruesome scene of the mattress. He says his mother-in-law was shocked, but his nephew loved the day.

An in-depth subplot detailing the origins of Pinhead (Doug Bradley) was scripted but deleted in preproduction due to last minute budget cuts. All that remains of this subplot is the film's prologue showing Capt. Spencer opening the box and transforming into Pinhead. - IMDb

Nicholas Vince (Chatterer) received a hook to the jaw while filming a scene involving his character being impaled on a swinging torture rack surrounded by the many hanging chains, it has been claimed that the cameraman stopped filming just at that instant. A scene in which the character receives his vision was removed from the final cut, causing some confusion at his introductory scene featuring him in his original eyeless guise.

Clare Higgins (Julia) hates horror movies. When she saw the first Hellraiser movie for the first time at the premiere, she had to leave after 10 minutes because it freaked her out so much. She has never seen the whole movie. - IMDb

At the time of the movie's release the MPAA had an agenda on "intensity of tone". As a result of this director Clive Barker had to make several cuts to the film - consecutive hammer blows, fingers entering flesh, S&M spanking between Julia and Frank, additional "thrusts" during the sex scene - all with the intention of watering down the overall impact of the piece. - IMDb
