Honestly, I think he needs glasses but is too vain to wear them. People with severe astigmatism may have to tilt their head to see without the image doubling.
That explains the weird tilt.
I dunno about the rest of it. Looks like he can't decide whether to bite or pee himself. Like a fearful dog that can't decide between submission and aggression
