if a common response to a req for a decision is "let's discuss at next meeting" and a common meeting agenda item is debriefing on things discussed at other meetings, is that bad? Or maybe just a difference in communication styles?

right up there with 'we do DevOps'.
No mate, you use a tool with DevOps in its name but you're still controlling everything from the top down.
@rpardee it's bad. symptomatic of procrastination and/or fear of the leadership team constantly overriding decisions.
I see it frequently with some clients and it always leads to delay and more cost.
One in particular springs to mind. They've dragged their feet so much that they've had to renew their data centre lease twice at a 7 figure sum each time.
