Most are caused by humans.
But lightening is a major source of ignition.
PG&E is deploying circuit breakers on the risky lines. They cut off power in 1/10th sec. So far they seem to be working.

Undergrounding lines is a solution, but even those don't last forever. In fact, they need to be replaced more often than tower-strung cables.
