In the aftermath of any major catastrophic event we hear the common complaint of "Why didn't somebody notice and say something sooner?"
Then we inevitably find out several people did notice and sounded alarms but those alarms were dismissed. It was true for both shuttle disasters with people choosing to resign rather than sign off on unsafe risks.
This, too, was likely avoidable if not for incompetence in the chain of command.

How about pressuring the cabin of an Apollo spacecraft with pure oxygen? How did any engineer not realize that was recipe for disaster?
