The blame game. It's always about blaming someone else rather than looking inward. Very tiring and unjust.

@CherNohio @th3j35t3r Don't know why this is, but assigning blame seems to always be the first (unofficial) human step in the quality improvement process before people can move on to actually doing something to fix problems or making things better. It serves no important purpose in the end.
